Abstract
We present our experience using the Kirschbaum's flap (in charretera) for reconstruction of scar retractions of the neck and face. This is a retrospective study of a series of 63 patients with burn scar contracture of the anterior portion of the neck and facial lower third, operated by the author between 1996 and 2012 in the context of the surgical campaigns developed around the country (Peru) in those geographic areas of greatest need for specialized medical attention. We use in all patients the "charretera" flap, raised from the lateral portion of the neck and the deltoid area. Patients were evaluated by physical exam and using pre and postoperative photos. We obtained improvement of the functional limitation of the neck and face areas in different degrees in all the operated cases, with total survival of the flap in 59 cases (93.65 %) and partial survival in 4 cases (6.34 %). We have no cases of total necrosis. The complication rate was low, mainly hematomas, 6 cases (52 %) and infection, 2 cases (3.17 %). The donor sites suffered hypertrophic scars in 22 cases (34.92 %). In conclusion, our experience using this surgical technique has proven useful in the treatment of burn sequels with scar retraction of the neck and face, so we considerer that Kirschbaum's flap is a safe method with low rate of complications.
