Abstract
Objective: To establish the relationship of precipitin antibodies anti-Sm, -RNP, - SSA/Ro and -SSB and their clinical significance in specific clinical manifestations of SLE. Methods: We measured anti-Sm -RNP, -SSA/Ro, and -SSB/La by ELISA, and also - ds DNA antibodies, in 126 A-A patients (121 female and 5 male patients). Their mean age was 30.1±10.2 years; disease duration was 11.5±8 years. Clinical manifestations were obtained from our computerized database. Results: Arthritis was present in 65.9%, skin involvement 43%, renal involvement 33.9%. Renal biopsy was only performed in 21(50%) of patients with renal involvement. Pleurisy was present in 30.2%, Raynaud's phenomenon in 28.6%, thrombocytopenia 14%, hemolytic anemia 10.3%, neurological involvement 9% of patients. Anti-ds DNA was positive in 35.5% and 45% of patients with and without renal involvement (p>0.05), -Sm 29.7%, -RNP 44.9%, -SSA/Ro 40.7%, and - SSB/La 15.3 % of patients. Patients positive for both anti-ds DNA and -RNP had significantly higher prevalence of pleurisy (p<0.05). Those positive for anti-RNP, -SSA/Ro, and -Sm had significantly higher prevalence of arthritis and Raynaud's phenomenon (p<0.05). Hemolytic anemia and leukopenia were less frequent in patients with positive anti-Sm, -RNP, and -SSA/Ro (p<0.05). There was a negative correlation between anti-ds DNA and -RNP positivity and photosensitivity (p< 0.05). Conclusion: There was no association between the presence of anti-Sm, -RNP, SSA/Ro, and SSB/La antibodies with lupus nephritis and CNS involvement. We found, however, significant association with arthritis and Raynaud's phenomenon. Anti-ds DNA antibodies did not correlate with renal involvement, but in combination with RNP was associated with pleurisy.
