PublicationCirculatory dynamics during high altitude pulmonary edema( 1969-01-01)Cardiac catheterization studies have been carried out in 2 young male subjects who experienced acute pulmonary edema after a brief sojourn at sea level, when they returned to their native town located at 14,200 feet above sea level. The investigation was performed at this altitude during the acute episode and was repeated after complete recovery. Further studies were made in 1 subject after prolonged residence at sea level. Severe hypoxemia, a marked degree of pulmonary hypertension and increased pulmonary vascular resistance were found during the acute pulmonary edema. These findings were associated with low cardiac output and pulmonary wedge pressure. The degree of pulmonary hypertension was significantly reduced after inhalation of 100 per cent oxygen. Following recovery, physiologic observations were similar to those seen in healthy residents well acclimatized to high altitudes. The data obtained suggest the presence of arteriolar constriction at the precapillary level due to severe hypoxia. Several factors may explain the severe hypoxemia observed in patients with high altitude pulmonary edema. Exercise and sleeping may decrease significantly the arterial oxygen saturation at high altitudes. Pulmonary edema occurred during exercise in our first case, and during sleep in the second. However, special susceptibility appears to be present in humans who develop pulmonary edema after rapid exposure to high altitudes. The role of a sudden rise of pulmonary artery pressure in producing pulmonary edema without elevation of pulmonary wedge pressure is not clear. © 1969.

PublicationBiological effects of repeated leukapheresis of patients with chronic myelogenous leukemia( 1973-01-01)
;McCredie K.B. ;Brittin G.M.Freireich E.J.Fifteen patients with chronic myelogenous leukemia were managed only with repeated leukapheresis for up to 26 mth. At each procedure approximately 10 l of blood were processed with a continuous flow blood cell separator over a 3 hr period. Five patients had intermittent leukapheresis (1-2 procedures/wk), and 10 of them had one or more series of intensive leukapheresis (4-5 procedures/wk). Addition of hydroxyethyl starch to the extracorporeal circuit was found to increase the removal of leukocytes fourfold. With intensive leukapheresis the leukocyte count decreased 80%, and the platelet count decreased 54% (mean values). With intermittent leukapheresis the corresponding figures were 70% and 35%. Thrombocytopenia was never clinically significant. All 15 patients experienced symptomatic improvement, and those with organomegaly had decrease in the size of the spleen and liver. Leukapheresis was not associated with morbidity, except that anemia became more severe and required periodic transfusions of packed red blood cells. Patients managed with leukapheresis did not enter bone marrow remission, and transformation of CML into blastic crisis was not prevented or delayed. -

PublicationMalignant melanoma of the head and neck(International College of Surgeons, 1973-12-01)31.6% of melanomas were found in the head and neck area. In this series of 67 cases, 31.3% were melanomas of the oral and nasal mucosa, a considerably higher figure than that reported in the literature. Melanoma of the eyes occurred at an earlier age than that of other localizations. The prognosis of this tumor is directly related to its skin, mucosal or ocular localization, to the presence or absence of metastasis and to the type of treatment. Survival for five years in melanoma of the head and neck skin was 75% for cases without ganglionary metastasis, primarily treated by us. This figure dropped to 16% in the presence of ganglionary metastasis. In melanomas of the oral and nasal mucosa the results of treatment were uniformly bad. The prognosis was much better for melanomas of the eyes than for the oral and nasal localizations but did not equal the results obtained in skin lesions. (Journal received: 5 Aug. 1975)

PublicationCharacterisation of a drug resistant strain of Plasmodium falciparum from SabahA strain of Plasmodium falciparum, transmitted in Beaufort District, Sabah, near the town Membakut, was isolated in July 1972 and transferred to the University of Maryland for characterization in non immune volunteers. At Maryland the Sabah (Kal.) strain, as it was designated, was transmitted efficiently by colonized Anopheles stephensi. Prophylactically, it was not suppressed by chloroquine or amodiaquine 0.3 g (base) or pyrimethamine 25 mg taken by volunteers at weekly intervals; however, proguanil 200 mg daily (but not 100 mg) was effective. Attacks of malaria attributable to this strain were radically cured by quinine 8.1 g (base) given during 8 days (10.7 g of quinine sulphate), but parasitaemia recrudesced after 3 and 5 day courses of chloroquine and after one of 3 courses of amodiaquine. Proguanil and pyrimethamine were ineffective curatively, and the latter drug did not inhibit oocyst development in the mosquito, whereas primaquine was an effective sporontocide and gametocytocide.
